Nurse Practitioner
POSITION SUMMARY
The ability to prescribe buprenorphine to improve the lives of at-risk populations and communities to combat the opioid epidemic and mental health medications for those who need support for mental health problems.
ESSENTIAL FUNCTIONS

Essential Functions:
Provide initial evaluation, diagnosis, treatment, and ongoing support for individuals with SUD and complex addiction; a comprehensive medical assessment
Thoroughly complete appropriate documentation for patient files and billing
Provide education and counseling to families/guardian(s) and patients regarding compliance with the program and medication compliance
Interpret tests results diagnosing any health issues and provide feedback to patients
Provide education regarding illness and prognosis to assist with patient and family goals to develop a comprehensive care plan
Provide continuity of care to patient including follow-ups regarding progress and medication, establishing communication with patient’s PCP when and if necessary
Evaluates lab results for abnormalities
Reviews and adjusts treatment plans based on patients’ progress in the program
Work with FCRC of Ohio staff to administer and establish the best medical therapy and behavioral health services for the individual patient

SKILLS & ABILITIES
Education and Experience: Must maintain required licensure, certifications, and CME credits,
Active and unrestricted medical license as a physician or nurse practitioner within the states of Ohio
Experience providing addiction medication for the socially and medically diverse population; experience with complex substance use disorders. Must have current DATA 2000 DEA-X waiver, initially at 30 for buprenorphine prescribing.
Computer Skills: Ability to use computers and office software, business office machines, tools, equipment as necessary to perform job duties. Experience with Electronic Medical Records and maintaining medical records for all patients.

Provide personalized, comprehensive addiction treatment collaborating with counselors, case managers, etc.
Performs other duties as assigned
Must be in good standing to practice medicine as a physician or nurse practitioner in Ohio and provide comprehensive quality patient care and referrals. One year of primary care experience and or substance use treatment experience preferred.
PHYSICAL DEMANDS
Individual may be required to sit for extended periods of time with occasional standing or walking.
Frequent use of fine motor skills to handle or feel objects, tools, or controls and ability to reach with hands and arms on a regular basis
Intermittently required to lift objects weighing up to 20 pounds
Consistently required to talk, hear, and/or speak
Specific vision abilities required by this job include close vision and the ability to adjust focus along with the ability to read, analyze, and interpret procedures and/or instructions
Some assignments may require ability to have or arrange for dependable means of transportation to other clinic locations, meetings, or community events
WORK ENVIRONMENT
Work is performed primarily in office, individual owned and insured vehicles, and outdoor settings, in all weather conditions, including temperature extremes, during day and night shifts. Work is often performed in emergency and stressful situations. The noise level in the work environment is usually quiet in office settings and moderate in other situations.
